Facts are vague, client was front passenger in friend’s vehicle traveling northbound
on I-605 when accident occurred. Client is not sure how Semi collided with another
commercial box truck and then they were struck. Client was transported to
LAC+USC Medical Center where she’s still hospitalized, X-Rays & CT Scans were
taken, and has a surgery scheduled on August 31st cause client suffered FX spine –
T-10, FX ribs on both sides – punctured lungs, and chest wall pain.
Plane was traveling on Holohan Rd when defendant veered into oncoming lanes, client immediately started honking her vehicles horn but defendant still collided head on with client. Defendant was arrested for DUI.
PC was merging lanes when DEF did not allow PC to merge and collided with PC driver-side rear.
Client and one other co-worker were assisting a Conco Pumping Technician as they propped a hose on their shoulders to avoid any kinks from forming and holding up on their shoulders. The tech. was controlling the amount of pressure that came out of the mechanical pump via a wireless remote. The signal became weak; and the tech continued to increase the pressure. Once the signal re-engaged, it caused client to be lifted up by the hose and caused him to land sitting down and his co-worker landed onto his head; sitting there stunned, client stood up and asked for medical attention.
Client was traveling towards Woodland, CA. He was on his usual Sunday afternoon
motorcycle drive. Client does not recall what happened next. He regained
consciousness and realized that he was laying face up on the floor. He remembers a
person named Hector who may have helped him. Next thing client remembers is the
paramedics strapping him to a board and transporting him to the hospital. There
was no communication with Def.
Client Making a left turn on Belleview St , Def tried to by-pass Client before light turned red hitting him on the driver’s side. Police arrived but no report was taken. This is a sub in
Client was at her Christmas party. as the party ended, and she started walking
towards the restroom she stepped on a rug which lifted up causing her to fall
backwards hitting her head on the floor. It seems that had mopped but did not dry
the floor good enough and just placed the rug on top of the wet floor. She stood up
and sat on a chair. A coworker came out of the stall and noticed what happened and
went to inform the manager who just came and asked her how she was doing but
did not take a report. Client shower him water underneath rug and he just
apologized. Coworkers brought her ice and everybody just left. She noticed a
camera on top of the wall. She was wearing boots.
Client was stopped at a red light when the defendant failed to stop and rear ended client.
Client was transported to Doctors Medical Center.
Client was stopped due to traffic when she was rear ended by defendant, the force
of the impact pushed her forward colliding with UPS truck. Client went on her own to
Washington Urgent Care.
Client was under the course and scope of employment, pouring cement at new
construction, when a person from JLS Concrete Pumping company operating a
concrete pump dropped a hose of pump weighing approx. 280 lb. on top of client,
hitting him on his left shoulder/neck area.
Client reported incident to employer. Employer referred him to Concentra where
he’s been treating and received an injection to left shoulder/ torn tendon, will
require surgery.
Per client, JLS Concrete Pumping was contracted by Swinerton Construction which is
the company in charge of whole construction site.
Client was traveling on US-101 when traffic came to a complete halt, seconds later
client was rear ended by defendant, force of the impact pushed clients vehicle
forward rear ending vehicle#3. Clients went on their own to Natividad Medical
Center in Salinas CA.
Client was traveling S/B on I-215 in the carpool lane stopped in traffic when
defendant traveling behind client failed to stop and rear-ended client at a high rate
of speed. Driver( Gustavo) sustained numbness and pain to his low back. Angelica
( passenger) sustained back and stomach pain as a result of the accident.
Clients were stopped eastbound on W. Mill St. when defendant traveling southbound
made a wide right turn causing collision. Clients were treated at the scene and
transported by ambulance to local medical center.
Client was on CA-91 On-Ramp in right lane when defendant merged into her lane
and struck her vehicle. Defendant dragged her vehicle for several feet.
Client was stopped northbound on CA-99 when he was rear ended by defendant who failed to stop.
Client was hired by his friend by his friend to assist him cut down some palm trees
at a private property located in Suisun City CA. Client was provided a ladder by the
homeowner and was approx 30-40 feet high when he fell and landed on the
pavement. Client was transported to North Bay Medical Center where he underwent
lumbar surgery. Client was hospitalized for over 8days.
Homeowner reached out and offered to pay client’s medical bills.
Client had lumbar surgery, right elbow surgery and other injuries.
Client was traveling northbound on Valle Verde Drive, defendant southbound on
Valle Verde Drive was stopped in left turn pocket, as client made a legal left hand
turn defendant for unknown reasons reversed and struck client on driver side doors.
Defendant then dragged client several feet. Defendant told client he didn’t see him
or hear him because he had his earphones on.
Client was driving for Rideshare transporting a passenger traveling north on US-101
when defendant traveling north on US-101 failed to stop rear ending client. The
force of the impact caused client to rear end vehicle #3.
Client was driving for Rideshare transporting a passenger when defendant made a left
hand turn and struck client on front driver side fender. Client’s vehicle was deemed
a total loss.
Client was driving for Rideshare transporting a passenger who was sitting on rear right
passenger seat. Client were traveling westbound on E. 7th St. in the #1 lane when
defendant traveling westbound on E. 7th Street failed to stop and rear ended client.
The force of the impact pushed client forward rear ending vehicle #3. Client was
treated at the scene.
Client was a passenger in an Rideshare when def rear ended them.
Client was traveling N/B on Hooper St. when defendant traveling S/B made a left
turn and struck clients front right bumper. Client and defendant exchanged
Insurance information.
Client was a passenger on Rideshare vehicle traveling on Presidio Street when Rideshare
driver collided with vehicle traveling on Pine Street. Client is not sure who is at fault.
Client was transported to local medical center.
Client blacked out, fractured sternum, fractures in cervical region, fractured rib,
headaches, shoulder and lumbar pain.
Client was a passenger in Rideshare Vehicle traveling north on SR-125 #! Lane when
defendant #2 lane merged left and struck vehicle client was in. Client was treated
and transported to local medical center.
Client was traveling N/B on S. Harbor Blvd., in the #2 lane, just passing W.
McArthur Blvd. at approx. 20-30 MPH when Defendant traveling N/B in #3 lane, who
was stopped behind a bus, suddenly veered into #2 lane, trying to pass bus,
colliding with right front side of client’s vehicle. Client was nervous and just wrote
down information Defendant gave him but forgot to write down name of driver, and
insurance company, didn’t take pictures.
Client was traveling westbound on SR-4 when he came to a complete stop due to traffic, defendant traveling westbound on SR-4 failed to stop and rear ended client. Client went on his own to Kaiser Permanente in Antioch CA. Client’s vehicle was deemed a total loss
Client was entering a mobile home park facing southbound behind defendant
waiting for gate to open when defendant reversed striking client. Both parties
exchanged information at the scene.
Client was traveling north on SR-99 when defendant traveling at high speed failed to
stop and rear ended client. Client was treated at the scene and transported to
Kaiser Permanente Modesto CA. Client has been treating with chiro since DOI.
Client was walking westbound on north sidewalk as she passed defendant’s home dog jumped up and attacked client. Client was knocked to the ground and homeowners came out and helped to get dog off of her. Client was transported to Kaiser Permanente.
Client has several punctures and in injured her back when she was knocked to the
ground
Client was a passenger on Company Pickup Truck traveling southbound on Antonio
Ave #1 lane. Defendant driving for Rideshare and transporting 1 passenger was parked
facing southbound when he tried making a U-Turn causing client’s driver to collide
with defendant. Client was treated at the scene.
Clients were passengers in a Rideshare traveling northbound on Vermont Ave. when Def
traveling in unk direction collided into Rideshare vehicle. Client’s went on their own to
Harbor UCLA.
Client was under course and scope of employment at the time of the accident for Rideshare, transporting a passenger. They were traveling E/B on Wilshire Blvd. in the #1 lane at approx. 30-35 MPH, when Defendant traveling W/B on Wilshire Blvd., attempted to make a left hand turn directly in front of client, causing collision. Client
had dash camera which caught accident. Defendant was arrested for DUI
Client was traveling on Rd near Lake Earl Drive when defendant veered into oncoming lanes colliding head on with client. Client was transported to Sutter Coast Medical Center and has been treating with his pcp. Defendant was DUI
Client was at the Buena Vista Landfill in Watsonville CA standing in his truck bed
when defendant driving commercial truck reversed and struck client’s vehicle.
Client was propelled into the air landing on a metal tool box.
Client was traveling on small street when he was rear ended by defendant. Client’s vehicle was totaled.
Client did not have auto insurance at the time if the accident
Client was traveling northbound on Frost Mar Place on a green light when defendant eastbound on Miramar Rd failed to stop at the red light colliding with client. Several witnesses exited their vehicles and rendered aid to client.
Client was a passenger in Rideshare when DEF traveling SB on SR-395 in lane #1 of 1
suddenly rear ended V-3 in same lane and pushed V-3 into the rear of Rideshare.
Client was driving for Rideshare transporting a client. Client was traveling southbound on
Columbia Street with right of way on a green light when defendant traveling
westbound on Ash Street failed to stop at the red light and struck client’s vehicle.
Client went on his own to Kaiser Permanente.
Client was Rideshare passenger, sitting on rear right seat. Client stated they were
traveling W/B on E. Riverside Dr. in the #2 lane at approx. 30 – 35 MPH with right
away when Defendant traveling N/B on S. Walker Ave. who had a stop sign, pulled
out directly in front of them causing Rideshare driver to collide with Defendant’s rear
passenger side. Client was treated at the scene.
Client was traveling westbound on Main Street in lane # 1 when suddenly rear
ended by defendant. Defendant left the scene. Client has full coverage UM.
Client went to a clinic in Centro De Rehabilitation.
Client was traveling on Bering Rd with the right of way on a green light when
defendant traveling on Brokaw Rd failed to stop at the red light and collided into
client’s passenger side.
